Your reporting effort will help significantly decrease the probability of serious incidents occurring in the future.

For sites in Ohio, Washington, Wyoming, and North Dakota, work with your PX team to find a provider

Injury/illness report: on-the-job injury and/or illness

Motor vehicle incident: auto accidents and accidents involving ATV's, golf carts, etc.

Property damage: fire, theft, water damage, vandalism, equipment damage

Cyber security and personal data: accidental disclosure of personal data, data breach, possible GDPR violation

Miscellaneous incident: any other incident that doesn't fit in a particular category

2 minute near-miss report: any event that has potential to cause, but did not end in injury or loss
